Long-term Services and Supports (LTSS) Improvement Tool

Background

Home and Community-Based Services (HCBS) play a vital role in supporting individuals with disabilities and the elderly to live in their own homes and communities. Total federal and state Medicaid long-term services and supports (LTSS) expenditures continue to grow nationally. The HCBS spending growth rate underscores the importance of monitoring service quality and outcomes for the people that we serve.

New Instrument

The Department of Human Services (DHS) is launching a new Long-Term Services and Supports (LTSS) Improvement Tool which will be available in the MnCHOICES Support Plan Application. The goal of the tool is to understand peoples’ experience with our services and improve service responsiveness. DHS aims to standardize data sources for federal reporting, inform measures for other evaluation efforts and evaluate lead agency performance.
